About Louise
My name is Louise Dwyer and I am the founder of Holistic Case Management Pty Ltd. I have over 30 years experience working in the disability and aged sector. I have completed my Bachelor of Psychological Science (with Distinction) at Deakin University,  Advanced Diploma in Disability Studies and Diploma Nursing.   I have extensive experience working with people with acquired brain injury, mental illness, including TAC clients, Supreme Court (Funds in Court) clients, National Injury Insurance Scheme Queensland (NIISQ), NDIS Participants, and people who are frail aged. I'm focussed on providing client centred care, and passionate about working with clients to meet their personal goals with a multidisciplinary approach. My priority is quality of care, respect and advocating for the rights of my clients.

Case Management
Develop goals with you and connect you to the services in your local area for you to reach and achieve them.
NDIS Support Coordination
Work with you on your NDIS goals and connect you to your choice of service providers. Build your capacity and strengthen your ability to make your own choices.
